Twice. Rough-in happens once the walls are open and before anything is closed up, and it has to pass inspection at that stage. Finish work comes back at the end to set fixtures and trim. Scheduling the rough-in late is the single most common way a remodel timeline slips, because drywall cannot go up until it passes.
Yes, where the work requires one, and City View schedules the inspection too. Moving a drain, adding a fixture, repiping or new rough-in generally needs a permit. Unpermitted work is the thing that surfaces when the house is sold or a claim is filed.
Usually, but how far and how cheaply depends on the drain. Supply lines are relatively easy to reroute; waste lines need fall, and that is governed by the floor structure rather than by preference. The plumber will tell you what the framing allows before the layout is settled.
